What a behavioral health treatment tracking app should track
Treatment tracking means knowing, for each client, where they are in care and whether they are getting better. A useful app does three things: it collects the same validated measures at set points, it shows them on one timeline, and it flags the clients who are getting worse or dropping out before anyone notices in a chart review.
Tracking points across a course of care
| Phase | When | Typical measures |
|---|---|---|
| Intake | Before or at the first visit | PHQ-9, GAD-7, plus DAST-10 / AUDIT-C or PCL-5 where relevant |
| Active treatment | Every 2-4 weeks | The same measures as intake, so change is comparable |
| Left early (against medical advice) | 48 hours, 7, 14 and 30 days after | Short check-in and re-engagement questions |
| Discharge | Last week of care | Intake measures repeated for the outcome score |
| Alumni / aftercare | Monthly for up to 12 months | Brief relapse-risk and wellbeing check-ins |
Signs a client needs attention
- A safety question answered positively — the clinician is alerted and the client sees the 988 Lifeline.
- A score rising five points or more since the last check-in.
- Two missed check-ins in a row, which often comes before dropping out.
- No improvement by the midpoint of the planned course.

Frequently asked questions
Do clients need to download an app?
No. Each check-in arrives as an emailed single-use link that opens on any phone or computer.
Can it track substance use and mental health together?
Yes. A schedule can combine measures such as PHQ-9, GAD-7, DAST-10 and AUDIT-C for the same client.
What does it cost?
Free for 5 patients forever, then $29.99 a year per clinician or $199 a year per 10 clinicians.
